2021-22 Alliance COVID Vaccination Policy

Players born BEFORE 2009, or are older than 12 years old in 2021, who intend to try out for London Jr Knights, or any other OHF sanctioned team, must be fully vaccinated or have an exemption from the Alliance. This policy applies to all approved coaching staff as well.  The Alliance website has instructions on how players can download a copy of their certificate of vaccination which is  registered under their provincial health card. All AAA team members and AA players still trying out with the LJK must provide proof of first vaccination before September 20.

USE THIS FORM TO UPLOAD YOUR VACCINATION DOCUMENTATION

Prior to September 20th or uploading, please bring your proof of vaccination or exemption to the arena withyou.

If you have not provided proof of your first vaccination certificate by Sep 20 you will not be allowed on LJK ice for AAA team functions or AA team tryouts. Final vaccination certificates must be uploaded here by Oct 17. Please refer to the Alliance Return to play hockey plan, where you can find details on the Alliance Vaccination policy.
