Police Record Check Forms

Attention Coaches and Managers;

It will be your responsibility to co-ordinate the collection and submission of all team staff police checks for the 2012-13 season.
If a team staff member's name is not on the updated list, a new police check will need to be submitted.
If you require a copy of the updated list please contact Pauline McCallum only through the Head Coach or Team Manager.



All staff who are not on the list shall go to the LJK web-site under Downloads, Team Documents and print the Police Record Check Form.
Complete it and then submit it in the LJK box located at Western Fair behind the Pete's Sports store.

There is also a link to the London Police Service Record Check information in case anyone has any questions about the new process.
Please use the form below that link called POLICE RECORD CHECK FORM.

Please be sure to attach 2 copies of photo I.D. with one being government issued and must have a photo, signature and address such as an Ont. D/L, Passport I.D. etc.

Do not VERIFY the form as it must be verified by the London Junior Knights Board approved person only.

*** Do not forget to sign the 2 places that it says applicant's signature. ***

Just a reminder that by our Policy it must be a LJK police check so one completed by another volunteer agency will not be accepted.

If you reside out of town, obtain the police check form from the out of town police agency. Complete it and place it in the Jr. Knights box at Western Fair.
Once the form has been reviewed and verified you will be contacted to make arrangements to pick it up and take it to your local police agency.
Once that process is completed you will be required to drop a copy of the completed police check back in the Jr. Knights drop off box located at Western Fair Sports Complex.
