For the 2014-15 season the Alliance has decided to do a pilot project with the Minor Atom AAA teams by collecting player's emergency/medical information online.
The company PrivIT Inc. is a privacy software company and the instructions are below.
Before completing your player's PrivIT health profile, we recommend you have the following information on hand:
- Family Medical History
- Personal Medical History
- Immunizations/Allergies/Medications
- Primary Health Insurance Information
To begin creating your player's health profile, select this link (
https://alliance.e-ppe.com) and follow the 4 steps of instructions.
- Click the blue "Register" button and fill out the registration information. This will be the email address and password you will use to access your player's PrivIT health profile in the future. Click Register. (Password must be 8 characters and contain a letter and number).
- Complete your (as in you the parent) Personal Details/Demographics section. You can navigate between sections by clicking the Next and Previous arrows or by clicking on the blue numbered components on the left.
You must complete 100% of each section before you can access the Take/Update Latest PrivIT Health Questionnaire section.
- On the Home page under Manage Family click the Manage Family Members button.
- Click Add New Member
- Enter your child's first (and last name if different).
- Click Add
- On the next screen you are able to copy PrivIT e-PPE family History data and demographic data from other family members. For instance the demographic information might be the same for you and your child and this information can be copied. In other cases if you have more than one child you are able to copy the PrivIT e-PPE family history data from one child to the next.
- Confirm the information you want copied and click copy.
- You will now see your child listed under Family Members and you can begin completing their health profile.
- To begin completing your player's health profile, click on the pencil in the PrivIT e-PPE Questionnaire column next to their name.
- A message will pop up notifying you that you will be editing information that will be associated with a particular association or organization. Click OK.
- Complete the demographic information section and click save and exit.
- You will be taken to the introduction page for the PrivIT Questionnaire. This message tells you a little bit about the questionnaire. Click Start.
- Complete all the sections of the health questionnaire. Although we recommend completing the questionnaire all together at once, you are able to "Save and Exit" to allow you to finish at a later date if needed.

IMPORTANT: In order for a section to be 100% complete you will need to answer all questions with an * and check the box at the bottom that states "I have thoroughly read all questions on this page and answered them to the best of my knowledge.*"
You will not be able to print your forms until this step is complete.
- After completing the PrivIT health profile, you MUST join your child to the team they will be participating on in the hockey year. To do this, select Manage Family Member from the home screen. Beside your child's name on the far right is Manage Teams. Select that and a list of each team in the association will appear. Select the box beside the respective team and click Done.
